For urgent referrals, please call. The online form below is best for non-urgent situations. We respond to all referrals as soon as we can.

What Happens Next

After you refer, here is what we do.

We reach out as soon as we can.

An ICADRC team member contacts the person directly. If they are hard to reach, we keep trying.

We listen first.

A short conversation to understand the situation. No qualifying questions before we listen. We just talk.

We connect them to what they need.

Programs they qualify for, partner organizations that can help, in-person appointments if it helps. We walk through the first step with them.

We follow up.

We check in to make sure the connection landed. We are your one phone number going forward.
