Short-Term Coordination.

Short-term help organizing the in-home and community-based services someone needs after a hospital stay, new diagnosis, or change in their living situation.

What it is

Someone to help line up the pieces.

Short-Term Coordination is short-term, hands-on help organizing the services someone needs to manage at home safely. Unlike long-term case management, this is a focused stretch, usually a few weeks to a few months, while a new plan gets up and running.

We are the connector between the person, their family, and the agencies actually delivering care.
What we coordinate

Common pieces of a plan.

In-home care

IHSS enrollment, private home-care agencies, scheduling, backup coverage.

Medical follow-up

Coordinating doctor visits, transportation, prescriptions, equipment delivery.

Meals & nutrition

Home-delivered meals, congregate meal sites, CalFresh, special diets.

Caregiver supports

Respite, training, support groups for the family member doing the day-to-day work.

Benefits & finance

Medi-Cal, IHSS, SSI, HICAP, utility assistance applications.

Equipment & safety

Durable medical equipment, home modifications, fall-prevention.

When it ends

You graduate when the plan runs itself.

Short-Term Coordination is short-term by design. Once the services are in place and the family knows who to call for what, we hand things off and stay available as a phone-call-away resource.
